Working with the advice of your Primary Physician , we provide hourly and 24 hour private care by RN/LPN. The home health nurse using holistic approach, will assess your immediate and extended environment.Teach you or the care givers about your medical condition, help collaborate with your primary physician and other disciplines to promote continuity of care. Supervise other support personnel.

 

Home health RN under the order of your primary physician will assess the level of care by you or care givers, adherence to medical regimen drug adverse effect and interaction and effectiveness of your pain medication and activity tolerance.Perform skilled assessment of the mental ,cognitive, nutrition, and hydration, integumentary, cardiovascular, Gastrointestinal and genitourinary system. Pay attention to respiratory system and evidence of infection like rashes, fever and discharge. Maintain IV line and administer IV medication as ordered by your physicians. Make and use the care plan. Co-ordinate services and communicate changes in your condition to other health professionals and the Agency. Educate you and your caregivers on your condition, safety and care.

 

With the advice of your primary physician , the licensed respiratory therapist will evaluate your respiratory status, provide teaching about respiratory equipment (oxygen and Nebulizer therapy) and provide respiratory care measures such us chest physiotherapy.

 

Home infusion RN under the order of your primary physician will carry out home assessment, on environmental safety such as electrical, and structural (stair way and barriers to bedside infusion pump). Sanitation hazards such running water and refrigeration.

Monitor vital signs, IV access sites, therapeutic response, side effect or adverse reaction to IV medications and your progress towards self care. Facilitate you towards self care. Communicate and collaborate with other health professionals involved in your care.
